If I am unpaid, do I skip the Vietnam work permit?

No as a default. The volunteer limb is unpaid work under a treaty with diplomatic or international-organisation confirmation. Unpaid labour for a café is not that confirmation.

Is a volunteer the same as a foreign intern?

No. The intern limb is a student or trainee internship. The volunteer limb is treaty-plus-confirmation. Do not swap the dossiers.
