An intern exemption is not a cheap employee

Can we hire a foreign intern without a work permit?

A student or trainee internship — including listed seafarer training — can be permit-exempt. A paid role that is actually employment is not that limb. Foreign workers generally need a permit.

If the internship lasts 18 months, is BHYT compulsory?

Compulsory BHYT for foreign employees keys to a definite-term labour contract of at least 12 months. An internship that is not that contract does not automatically inherit the BHYT gate — and a disguised employment contract does.

What if they are just ‘helping for three months’?

Working without a permit where one is required can mean forced exit or deportation. Three months of labour is not a synonym for the student-intern exemption.
